Doctors can complete the application form electronically via eHealth, print and post the duly completed application form together with all supporting documents to:

If not yet eHealth connected

Doctors who are not yet eHealth-connected can download and fill in manually the Programme forms and submit by post the requisite forms for eHealth and Programme application together with their supporting documents to the DH’s Programme Office for facilitated processing.
